Who is Eligible:
Certain full-time Administrative and Managerial, Professional, Supervisory, Confidential and Cooperative Extension employees of the university enrolled in Public Employees' Retirement System (PERS), Police and Firemen’s Retirement System (PFRS), Civil Service Retirement System or the Federal Employment Retirement System (FERS). The employee must:
- Hold at least a 10-month appointment
- Receive an annual salary of at least $15,000
- Must be an active employee working a minimum of 35 hours a week
Enrollment:
You must enroll at the time of your initial eligibility. If you enroll after that period, proof of insurability must be submitted. You are initially eligible when:
- You are a new hire (coverage is effective the first day of the month following a full three-month waiting period)
- You have a job change from an ineligible position to an eligible position (coverage is generally effective the first of the month following the job change)
Benefit Amount:
66 2/3% of monthly earnings not to exceed $5,000 per month

- Complete a UNUM LTD Enrollment form.
- If you are enrolling 31 days after your eligibility date, an UNUM Provident LTD Evidence of Insurance form must be completed.
- Submit form(s) to University Human Resources (UHR)
